Instructions

1. Use the table below to determine which award level you are applying for.

2. Enter the name of the person you are nominating and fill in all required fields.

3. Describe the action or behavior you are recognizing in detail. Higher level awards require financial justification.
Ex. “In January, John Smith saved the company $1,000 by identifying unnecessary process steps in hydrostatic testing”.

Award Levels
Emerald | $500 -$1,000
Recognizing outstanding behavior. Requires CEO approval.
Note: If you are unable to quantify the benefit to Hudson, please consider nominating the employee for the Gold or Silver Award.
Gold | $100 – $500
Manager initiated award to recognize extraordinary effort or initiative in performance of responsibilities.
Silver | $50 – $100
Peer-to-Peer recognition for someone ‘caught in the act’ going above and beyond or modeling good behaviors for others.
