Employee Recognition Nomination Form


1st – Use the table below to determine which award level you are applying for.

2nd – Enter the name of the person you are nominating and the additional information.

3rd – Describe the action or behavior you are recognizing in detail. Higher level awards require financial justification. Example “In January, John Smith saved the company $1,000 by identifying unnecessary process steps in hydrostatic testing”.

Award Levels
$500 -$1,000. Recognizing outstanding behavior and quantified benefit to Hudson. Requires CEO approval.
$100 – $500 Manager initiated award to recognize extraordinary effort or initiative in performance of responsibilities.
$50 – $100. Peer-to-Peer recognition for someone ‘caught in the act’ going above and beyond or modeling good behaviors for others.
